2012-2013 NEW IMMUNIZATION REQUIREMENT
ATTENTION PARENTS OF SIXTH AND NINTH GRADE
The Illinois State Board of Education in conjunction with the Illinois Department of Public Health has instituted a new health requirement rule. All students entering sixth and ninth grades must provide proof of having received the Tdap vaccination along with the required school physical.
Students in 6th and 9th grade without one of the following will be subject to exclusion:
1. Proof of Tdap vaccination
2. An approved medical or religious exemption on file with the school,
Please ensure that your student receives this immunization prior to the first day of school, Wednesday August 15, 2012.  If you have any questions please contact your school nurse.
